This is a C6 (D on top) version of the tune 'Londonderry Aire', also known as the song 'Danny Boy'.

The famous Winston/Keith book has 3 versions of this tune in it - one of which is for C6, but it doesn't bear much resemblance to this arrangement.

There's a tricky bit where you have to half pedal on 4 (shown as '4*') as an afterthought while sliding up to get a dominant seventh. If you can't do this then just leave out the 8th string.

At the end, be sure to 'stop' the 5th string before sliding up.
